Who's left to Clerk?

There is a particular kind of exhaustion that doesn't show up on a rota.

It lives in the doctor who passed their finals, survived foundation, earned their specialty training number and is now quietly updating their LinkedIn. Not because they failed. Because the system did.

The numbers are not new. Medical school places expanded without a corresponding expansion in training posts. Highly qualified doctors are entering a bottleneck they were never warned about. Some wait years in trust grade posts, doing the work of registrars without the recognition. Others leave altogether - for Australia, for industry, for anything that offers a future they can actually plan for.

We are losing people who were built for this.

There is a word for what junior doctors do in their first years that most people outside medicine don't fully understand. To clerk a patient is to sit with them. To take the history, examine, document, synthesise. It is the foundational act of clinical medicine - unhurried attention at the point of contact. It is where diagnosis actually begins.

The word is quiet and unglamorous. It doesn't make headlines. But everything else depends on it.

Clerked exists because of the people who do this work.

Not the version of medicine that lives in press releases or policy documents. The version that happens at 2am on a take, in a bay with four confused patients and one functioning computer. The version held together by people who chose this life not because it was easy but because it genuinely mattered to them.

We are not interested in performing that for anyone. We are interested in wearing it - the language, the shorthand, the identity - quietly, correctly, without explanation.

Because if you know, you know.

The conversation about training posts and workforce planning will continue. It should. The structural failures are real and they deserve serious scrutiny.

But this is not that conversation.

This is for the ones still here. Still clerking. Still showing up for a system that is, at best, learning to value them.

You were built for this. The badge, the bleep, the 6am ward round no one told you was happening.
